HOME CARE PACKAGES
Exercise Physiology – Home Care Packages (HCP) and Aged Care Programs
Accredited Exercise Physiology services are available under Home Care Packages (HCP), Short-Term Restorative Care (STRC), and the Commonwealth Home Support Programme (CHSP). These programs allow older adults to access exercise support aimed at promoting independence, mobility, and quality of life — delivered in the comfort of home or community settings.
Exercise Physiology can play a vital role in helping older adults maintain function, manage chronic conditions, and recover from injury, with a focus on staying active, safe, and independent for longer.
Exercise Physiology Can Support With:
Strength & Bone Health: Individualised programs targeting bone density and muscle strength to help reduce the risk of osteoporosis and sarcopenia
Chronic Disease Management: Exercise strategies for managing diabetes, cardiovascular disease, pulmonary conditions, cancer, obesity, and depression
Pain & Injury Support: Rehabilitative exercises for chronic or acute musculoskeletal conditions, including hip, knee, shoulder, and back pain
Falls Prevention: Balance and coordination training to reduce fall risk and improve stability
Hydrotherapy: Low-impact aquatic sessions to relieve joint pressure and improve movement
Cognitive Engagement: Functional tasks and movement drills to support memory, reaction speed, and coordination
Daily Living Function: Improve capacity to complete everyday tasks — like standing from a chair, gardening, shopping, or playing with grandchildren — with more confidence and less pain
Exercise Physiology sessions are available one-on-one or as part of group programs, and can be delivered via mobile service across Perth — whether at home, in a local hydrotherapy pool, or in supported living environments.
